St John Bosco Gymnastics Club

Annual SJBYC and Gymnastics NSW Registration fees go to Gymnastics NSW, St John Bosco Youth Centre and St John Bosco Gymnastics.

As Parent/Guardian I hereby give permission for my child/ward to participate in gymnastics with St John Boco Youth Centre Gymnastics (The Club) and that gymnastics is a dangerous sport.

I request that you accept this application for membership and tuition.  I declare that I am legally bound for myself, my heirs/executors/administrators to waive and release the St John Bosco Youth Centre Gymnastics Club, Committee, Head Coach, Instructors, Club Members, Volunteers and their agents and or representatives from any and all claims or rights claimed for damages for injuries or loss suffered by me or by my child/ward, whether these claims or rights claimed are of a personal nature or otherwise and are incurred under any of the following circumstances, directly or indirectly in receiving tuition, travelling to and from any such periods of tuition, competing in events and training sessions.

I acknowledge that the instructors may be unqualified and that they operate under the direction of the Head Coach, or qualified Coach.

I authorise the St John Bosco Youth Centre Gymnastics Club Head Coach /Instructors/Committee/Volunteers to seek medical assistance and treatment in the event of a medical emergency involving my child/ward and agree to pay all expenses incurred as a result of medical assistance/treatment.

I agree that under the club privacy policy my Child’s information will be passed on to our governing body’s being St John Bosco Youth Centre, Gymnastics NSW and Gymnastics Australia.

I agree to allow photos or video images of my child be taken and used by the club for training and advertising purposes.

I agree to the Club code of conduct, including my responsibility to drop off and collect my Child/Ward from inside the Gym for safety reasons and the Clubs duty of care.
